Annual Medical Form:
A new medical form needs to be filled out for everyone (Scouts, Parents, and siblings) that will be attending any Pack events.

Please print the completed form out and bring it to a den or pack meeting!
Everyone is invited, bring your siblings!
Cost: All Scout over the age of 7 and their adults are invited for $75.00 per person; Siblings under 7 cannot attend per USS Lexington rules!

SPECIAL NOTES:
Volunteers under the age of 14 must complete a Minor Waiver Form signed by a parent or guardian and must be accompanied by a parent or guardian
throughout the shift.
All volunteers under the age of 18 also need a completed Minor Waiver Form signed by a parent or guardian.
Volunteers over the age of 18 must complete an Adult Waiver Form for themselves.
A copy of the Adult & Minor Waiver forms can be downloaded by clicking the RESOURCES tab on your Volunteer Profile page. Print these out and deliver directly to HFB upon your arrival the day of the event.
